Payment is due in full at the time of service. A credit card is required on file at the time of scheduling to confirm your appointment.
We reserve appointment time specifically for you. Out of respect for your care and the care of other patients, we ask that cancellations be made as early as possible.

These fees are charged to the card on file. Cancellations due to a medical emergency requiring emergency department care for you, your child, or your spouse/partner will be waived upon submission of supporting documentation.
Surgical fees are due in full 14 days prior to your surgery date. Payment is collected via the card on file or by approved alternative arrangement.
Failure to submit payment by this deadline will result in cancellation of the scheduled procedure. In the event of cancellation due to non-payment, the preoperative visit fee of $500 will be billed separately and is non-refundable.

A surgical fee will not be forfeited in the event of a qualifying medical emergency — defined as a condition affecting the patient directly that renders the scheduled surgery unsafe or medically contraindicated.
In qualifying cases, surgery will be rescheduled at no additional cost. Supporting documentation is required.
